1. Please complete the entire form. Sign and date the affidavit and send with check for dues to

Each family member must fill out a separate form. Make checks payable to CRCC. Enclose a self-addressed, stamped envelope and you will receive, by return mail, your 2010 membership sticker to place on the top right corner of your AMA card.
2. Initial club membership fee is $130.00 ($50.00 initiation and $80.00 1st year dues). Dues for members 65 and over are $110.00 ($50.00 initiation and $60.00 1st year dues). Dues for active military personnel and those 17 and under are $35.00 yearly with no initiation fee. Add $20.00 for each additional flying family member. Membership is valid through November 30, 2010.
3. Yearly renewal dues will be $80.00 for regular membership, $60.00 for members 65 and over, and $35 for active military personnel and those 17 and under. Add $20.00 for each additional flying family member. All renewals must be paid by November 30th or a $20.00 late fee will be applied. After December 31st unpaid members will be removed from the roster and considered new members.
